The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.
Responsibilities
The Employment Coach employs a variety of strategies, approaches and techniques to manage a member's health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.
Essential Role Functions and Responsibilities
Assistance to support each participant enrolled in the Workforce Development Program in planning for the future (e.g., education, training, financial counseling)
Assist with engaging the participant in and maintaining meaningful work (e.g., job support and retention coaching)
Seek to build access to a network of Community Based Organizations with expertise in providing these services
Will connect participant to resources across the community to address any unmet needs that present barriers to finding and retaining employment
Will connect participant with comprehensive career coaching services to identify their strengths and goals, hone the skills needed to succeed in the workplace, and match them to appropriate job openings
Identifying support services that address life challenges and unmet needs are key to securing and maintaining a job
WORKING FROM HOME - Temporary Due to COVID-19 emergency. Please note this permanent position allows you to work at home until further notice. This position will return to a physical office at a future date to be determined by leadership. See the Additional Information section below.
Required Qualifications
Bachelor's degree in human relations, social work, rehabilitation counseling or related field
2 or more years of experience in workforce development, human services, and/or counseling environments, such as a workforce development program, workforce staffing or a community rehabilitation agency providing services to individuals with disabilities and/or disadvantages
Background in Coaching and Development
Knowledge of Microsoft Office Word and Excel
Exceptional verbal/written communication and interpersonal skills
Ability to use a variety of electronic information applications/software programs
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
Ability to travel within the Louisville, KY are and surrounding counties
Must have the ability to provide a high speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required
A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Preferred Qualifications
Master's degree
Background working with Medicaid population
Prior experience working with job placements, on-the-job training, vocational training institutions and/or career support services
Previous experience in recruiting and/or employment coaching related experience (Full cycle recruiting or employment skills training in resume writing or interviewing skills)
Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems
Knowledge of community health and social service agencies and additional community resources
