Description
The Medicaid Housing Specialist (Care Management Support Professional 1)contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Professional 1 work assignments are often straightforward and of moderate complexity.
Responsibilities
Humana's Louisiana Medicaid Housing Specialist will support clinical care teams in order to help Medicaid enrollees find, and keep, safe and stable housing. They will be responsible for ensuring that enrollees transitioning from facility to community are connected to appropriate housing resources, including but not limited to, referral of potential enrollees to Humana's Permanent Supportive Housing liaison for application to the Louisiana Permanent Supportive Housing program.
Essential Functions and Responsibilities
Serve on the Multi-disciplinary care team with case managers and other care team members to provide housing resources as needed
Consult directly with enrollees or authorized representative to identify safe, affordable housing, based upon enrollee needs
Identify and maintain list of emergency, transitional, and permanent housing resources throughout Louisiana
Help build innovative partnerships between Humana and housing-related community-based organizations to improve enrollee health outcomes
Coordinate Humana's medical respite and eviction diversion programs
Advocate for enrollees with landlords and housing agencies as needed
Coordinate with community organizations, government agencies, and property owners to secure living arrangements, negotiate rent, deposits, and move-in terms on behalf of enrollees
Serve as a liaison between Humana and housing agencies, LDH, community-based organizations, and providers regarding issues related to housing needs for enrollees
Coordinate with Humana's housing strategy partners
Collect and analyze performance reports on Humana's housing support functions to monitor adherence with benchmarks, identify opportunities for process improvement, and develop recommendations for leadership
Coordinate with the Chief Compliance Officer to submit semi-annual updates to the Louisiana Department of Health (LDH) and network providers regarding enrollee support services and effectiveness
Required Qualifications
Minimum of two (2) years' work experience for a public agency or related work in social services, community services, or case management fields
Knowledge of, and expertise in, Louisiana-specific demographics, organizations, housing, and geography
Demonstrated ability to constructively work with persons in crisis situations
Must reside in LA
Preferred Qualifications
Bachelor's degree in human services, social work or counseling, public health or administration,
Bilingual - Spanish or Vietnamese preferred
Experience in housing advocacy and/or support
Knowledge of and experience with relevant Housing and Urban Development (HUD) regulations and affordable housing financing
Previous experience tracking, analyzing, and reporting on State and Federal changes to housing and community development programs
Workstyle: Combination work at home and field with local travel to meet with members
Location: Must reside in Louisiana
Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM Central Standard Time, over-time may be requested to meet business needs.
Tuberculosis (TB) screening: This role is considered member facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Covid Policy: Humana and its subsidiaries require vaccinated associates who work outside of their home to submit proof of vaccination, including COVID-19 boosters. Associates who remain unvaccinated must either undergo weekly negative COVID testing OR wear a mask at all times while in a Humana facility or while working in the field
Driver's License, Reliable Transportation, Insurance This role is part of Humana's Driver safety program and therefore requires an individual to have:
a valid state driver's license,
proof of personal vehicle liability insurance with at least 100/300/100 limits,
and a reliable vehicle.
