Case Manager-General Outpatient Program (Full Time)
Location: Woonsocket, RI
Community Care Alliance is seeking to add a motivated, task oriented, enthusiastic case manager to their Integrated Health Home Team! This position is interwoven into the Adult General Outpatient Department thus there will be no requirement to work evening or weekend hours. Our mission is to provide individuals with wraparound supports and services in order to improve and stabilize functioning so clients can achieve independence!
Typical job duties include
Coordinating with a multidisciplinary team consisting of case managers, nurses, substance use specialists, clinicians and psychiatrists to enhance client care.
Providing transportation to individuals, as necessary, such as to medical visits, court appearances, to pick up prescriptions etc.
Providing assessment and referrals to various levels of care based on presenting concerns for mental health and/or substance use
Working with clients to explore and access community resources, completing applications for benefit, and housing etc.
Sessions occur within the agency, community and individual client homes

Candidates with, at least, an associate's degree in a Human Services or related field
Candidates with an approved driving record and reliable transportation
Education/Licensing Requirements: AA in a human services field required; BA/BSW in a human services related field preferred. Valid driver's license. Position requires you to work towards CCSP after 6 months of employment (sponsored by CCA)
